Below are rules for the facility.
Anyone participating at events at the facility must have a liability waiver form on file. If you are currently participating in an FSCI program, then you already meet that requirement because that was a prerequisite to joining the program.
For example, travel players must submit a PPA prior to participating. The PPA is the liability waiver form. For those that are not currently in any FSCI programs, you must sign the liability waiver form below:
1-NO CLEATS, Use indoor shoes or flats.
2-NO GUM OR FOOD in the playing surfaces.
3-NO PUNTING SOCCER BALLS.
4-NO ANIMALS OR PETS
5-All players must have a participation consent form on file
